私がオリジナルのケーススタディを作成するに至った背景について、まず日本語で、続いて英語でお話しさせて頂きます。少し長い内容になりますが、心を込めてお伝えしたいと思い、このメッセージをお送りします。

Allow me to share the background of how I came to develop my original case studies — first in Japanese, then in English. The message is a bit long, but I’m writing it with all my heart to sincerely share my story with you.

私のオンラインコースをご検討中の皆さまへ

このコースに登場するケーススタディの問題、解答、そして解説は、すべて私自身が作成したオリジナルです。Next Generation NCLEXの形式に則っており、2022年9月に私自身が経験した交通事故をきっかけに着想を得て作成しました。この事故により、私は普段勤務している病院のメディカル・サージカル病棟に入院することになりました。事故は、信号のない四車線の幹線道路を通勤中、左折しようとした10代の運転者が安全確認を怠ったことにより発生しました。幸いなことに、救急外来（ED）、回復室（PACU）、そして病棟の同僚たちから素晴らしいケアを受けることができました。

ケーススタディを可能な限り現実的かつ実践的なものにするため、私の看護師としての臨床経験に基づいて患者シナリオを構築しました。

なぜ私がこの教材を自ら作成したのか

私は、学生がNext Generation NCLEXに合格できるよう支援するため、自らオンラインコースを立ち上げました。教材を完成させるまでには、2か月半を要しました。受講を検討している方々の中には、入会前に無料のケーススタディを試してみたいというご要望もありました。既存の教材を使うことができなかったため、すべてゼロから自作することを決意しました。提供するケーススタディはすべて100％オリジナルであり、教育的価値が高く、文化や医療制度の違いを乗り越えることを目的としています。

十分な準備のために、Donna D. Ignatavicius著『Developing Clinical Judgment for Professional Nursing and the Next-Generation NCLEX-RN Examination』（ISBN: 978-0-323-71858-5）を繰り返し学習しました。頻繁に読み込んだ結果、教科書はすっかり擦り切れてしまったほどです。私はイグナタヴィシウス氏と、出版社エルゼビアのエグゼクティブ・コンテンツ・ストラテジストであるリー氏にも直接連絡を取り、教材内での使用許可をいただきました。ただし、受講生にはこの教科書（エルゼビア出版）を購入していただく必要があります。

このプロジェクトに込めた想い

私の最大の動機は、アメリカで看護師になるという夢を持つ日本人を支援することです。多くの方にとって最大の壁は、Next Generation NCLEXの合格です。英語で医療知識を学ぶことの大変さは、私自身よく理解しています。十分な準備なしで挑戦するのは、非常に大きなエネルギーと自信が求められます。

もう一つの目標は、すでに日本で数年の臨床経験を持つ看護師の方々をサポートすることです。私は、まずはCertified Nursing Assistant（CNA：認定看護助手）としてアメリカの医療現場に入ることを勧めています。一部の方はCNAとして働くことを「後退」と感じるかもしれませんが、私はこれを大切なステップと捉えています。CNAの経験を通して、医療英語に慣れ、アメリカの医療システムを理解することができます。日本の看護師はすでに高い臨床スキルを持っていますが、医療英語の自信を持つには時間と現場経験が必要です。適切なサポートがあれば、NCLEXに自信を持って臨むことができるようになります。

残念ながら、日本の英語教育はこれらの課題に対応するには十分ではないと私は感じています。だからこそ私は、単なる学習支援にとどまらず、励まし、文化的理解、そして実践的な成功への道を提供するこのオンラインコースを作りました。

ご存じの通り、日本の正看護師（RN）はビザの関係でアメリカでCNAとして働くことは通常認められていません。これは大きな障壁です。しかし私は、このビジョンを理解し、支援してくれるアメリカの企業が必ず存在すると信じています。たとえ最初はCNAとしてのスタートであっても、日本の看護師が持つ価値を認めてくれる特別なパートナーを探し続けます。これは、関わるすべての人にとって価値あるチャンスだと確信しています。

最終的な目標は、Next Generation NCLEXに合格し、アメリカでRNとして活躍するまでの道のりを、誠実に寄り添いながら支援することです。

そしてその中心にあるのがこのオンラインコースであり、その目的は明確でシンプルです：

日本の看護師および看護学生が、アメリカの医療現場で成功するために必要なスキル、自信、そして支援を提供すること。

その目標のために、私は自らオリジナルのケーススタディを作成しました。

To Those Considering My Online Course,

I developed all the case study questions, answers, and rationales in this course entirely by myself. They are written in the style of the Next Generation NCLEX and were inspired by my personal experience following a car accident in September 2022 — an incident that left me hospitalized on the medical-surgical floor where I usually work. The accident occurred on my way to work when a teenager failed to yield while turning left on a major four-lane street without traffic signals. Thankfully, I received outstanding care from my colleagues in the ED, PACU, and on the unit.

To make these case studies as realistic and practical as possible, I based them on actual clinical experiences I’ve had throughout my nursing career.

Why I Created These Materials Myself

I created my own online course to help students prepare for and pass the Next Generation NCLEX. It took me two and a half months to complete the materials. Some prospective students requested a free trial case study before enrolling. Since I cannot use content created by others, I chose to develop everything from scratch. Every case study I provide is 100% original, educational, and designed to bridge the cultural and healthcare system gaps that many international students face.

To ensure I was well-prepared, I studied Developing Clinical Judgment for Professional Nursing and the Next-Generation NCLEX-RN Examination by Donna D. Ignatavicius (ISBN: 978-0-323-71858-5) extensively—so that my textbook became worn out from frequent use. I even contacted Ms. Ignatavicius and Mr. Lee, Executive Content Strategist at Elsevier, and they kindly gave me permission to reference the textbook in my course, with the understanding that each student purchases a copy of Donna’s textbook, published by Elsevier.

The Vision Behind This Project

My greatest motivation is to support Japanese individuals who dream of becoming nurses in the United States. The biggest obstacle many face is passing the Next Generation NCLEX exam. I know how overwhelming it can be to study complex medical content in English. It takes tremendous effort and confidence—especially without the right preparation.

Another goal of mine is to support Japanese nurses who already have a few years of clinical experience in Japan. I hope to help them enter the U.S. healthcare system by first working as Certified Nursing Assistants (CNAs). While some may view CNA work as a step backward, I see it as an essential stepping stone. It offers exposure to medical English and helps nurses gain a better understanding of how healthcare functions in the U.S. Many Japanese nurses already possess excellent clinical skills—but gaining confidence in English, especially medical terminology, takes time and practical experience. With the right support, they can approach the Next Generation NCLEX with confidence.

Unfortunately, English education in Japan often falls short in preparing individuals for these challenges. That’s why I created this online course—to offer not just academic support, but also encouragement, cultural understanding, and a practical pathway to success.

As you may know, Japanese RNs are typically not eligible to work as CNAs due to visa restrictions. This remains a major obstacle. However, I truly believe that there are U.S. companies out there willing to support this vision. I will continue searching for those special partners who recognize the value Japanese RNs can bring—even if they start their journey as CNAs. I see this as a meaningful opportunity for everyone involved.

My ultimate goal is to walk alongside these dedicated professionals—to guide, support, and empower them as they prepare for and pass the Next Generation NCLEX and eventually become licensed RNs in the United States.

At the heart of it all, I created this online course with a clear and simple purpose:

To give Japanese nurses and nursing students the tools, confidence, and support they need to succeed in the U.S. healthcare system.

Creating original case studies was a key step in achieving that goal.
